Western Reserve Care Solutions provides basic life and AD&D insurance for employees and offers voluntary insurance options for employees and their dependent(s).

Basic Life and AD&D Insurance

Life insurance is an important part of your financial wellbeing, especially if others depend on you for support. Western Reserve Care Solutions provides basic life and accidental death and dismemberment insurance at no cost equal to 1 times your base annual earnings, rounded to the next higher $1,000, up to a maximum of $50,000. Coverage is automatic; you do not need to enroll, but you must designate a beneficiary.

Voluntary Life Insurance

You may choose to purchase additional life coverage for yourself and your dependents at affordable group rates. Rates are based on age and the coverage level chosen

Employee
  • Increments of $25,000
  • Up to a maximum of the lesser of 5x pay or $500,000
  • Guaranteed issue up to $150,000 for new hires
Spouse
  • Increments of $5,000
  • Up to a maximum of $100,000, not to exceed 50% of employee’s Voluntary Life Benefit
  • Guaranteed issue up to $50,000 for new hires
Children
  • Child Under 15 days: $100
  • Child 15 days to 6 months old: $1,000
  • Child more than 6 months old: Options of $1,000, $2,000, $4,000, $5,000 or $10,000
  • Guaranteed issue up to $10,000 for new hires

Voluntary Disability Insurance

Voluntary disability insurance provides income replacement should you become disabled and unable to work due to a non-work-related illness or injury.

Voluntary Short-Term Disability

60% of your predisability weekly earnings, to a maximum of $2,000 per week for the first 12 weeks of a disability after the 7-day waiting period. Benefits are offset by the other sources of income, such as Social Security and Worker’s Compensation.

Voluntary Long-Term Disability
  • 60% of the first $16,667 of your predisability earnings, to a maximum of $10,000 per month if you are disabled and are unable to work for more than 90 days.
  • Benefits are offset with other sources of income, such as Social Security and Workers’ Compensation.

Evidence of Insurability (EOI) Requirements

Voluntary Life:EOI is required if you are requesting coverage for an amount that exceeds $150,000. If you are not currently enrolled in the plan, or you are currently enrolled in the plan for a benefit less than $150,000, you may request coverage up to $150,000 without providing EOI.

EOI will also be required if you are requesting coverage for an amount that exceeds $50,000 for a spouse.


STD and LTD: EOI is required if you did not elect this coverage when you were first eligible as a new hire.

If you do not submit the EOI form to MetLife, you will not be covered under the benefit.
